Empower providers to improve vaccination rates by delivering real-time access to patients’ immunization histories at the point of care. Using the integrated “vaccine forecaster”, Pharmacists can see a patient specific guide to due or past due vaccinations and make recommendations to get their patients up to date.

$30 per Provider per Month x 12 months subscription = $360.00 for one provider for an entire year with automatic yearly renewals.

Also, a one-time implementation and on-boarding fee of $2,000 will be charged.

*This is for the yearly fee for one store for a year. If you have more than one store, please adjust the quantity of this product to reflect the number of stores you want immunization reporting to be activated in.

ONC Health IT Certification Disclosures

This is a software as a service subscription. STC shall report immunization transactions received from the customer to the Applicable State Immunization Information System(s) and return patient immunization histories and vaccine recommendations when available.The  functionality provided through ‌ImmsLink®,is dependent on state specific functional. The initial term is for 1 year effective on the date payment is made. Subscription will automatically renew for 1 year unless terminated by either party in writing 60 days prior to the end of current term.Changes in terms, conditions and pricing are at the sole discretion of STC. 

Data Rights.  Company expressly acknowledges that as part of this Agreement, STC will have access to various data including, but not limited to information about patients’ health, immunization histories, family history, personally identifiable information ,and any and all other data and information supplied by Company to STC during the term of this Agreement (collectively “Data”).   Company grants to STC a nonexclusive, irrevocable, right to use, reproduce, and create derivative works of the Data. The Data will be considered Confidential Information as defined in this Agreement and STC agrees to abide by the obligations relating to Confidential Information as it relates to the Data for as long as the Data meets the definition of Confidential Information as set forth herein.
